There are forms available for:

• Article/Book Request (Word Form) - to copy and paste in request information before printing, scanning and forwarding by email or fax - signature required (NB date format for signature box is ddmmyy)
  
• Article/Book Request (PDF Form) - to print off and complete, scan and forward by email, fax or internal post - signature required

If you want to submit an item to the Staff Publications database, please complete the Staff Publication Submission Form.  Please note you should only submit publications that have been published during your time employed by BSMHfT.

To join the Library, please print off and complete the Library Registration Form and return it to your nearest BSMHfT Library.
