Community Care

Community teams offer support to people within their own home or at centres within the local community.

They are usually made up of both mental health and social care staff so that people who are seen by the teams have both their health and social needs supported. This has been found to help people cope more effectively with mental health problems.

Services offered to adults of working age in the Trust include:

Community Mental Health Teams - Community Mental Health Teams (CMHTs) are groups of mental health and social care staff that support people experiencing severe and enduring mental illness.

Pyschology Services and Psychological Therapies - Psychologists are trained professionals who assess and treat people who experience mental health problems.

Psychiatric Liaison - All patients who go to the Accident and Emergency Department of a General hospital with a mental health problem or having attempted deliberate self harm will receive a comprehensive mental health assessment from Psychiatric Liaison Team.

Primary Care Mental Health Services -  This service provides a primary care mental health service, based on psychological therapies in GP surgeries.
