Take part in our quality account survey

Each year, our trust is required to produce a quality account as part of our annual reporting. The quality account provides to the public a review of quality of our services for the previous financial year.

As a part of this process, we are also required to agree priorities for improvement for the next financial year (2012/13).

We have been working to decide on what these priorities should be, but we would also really like your input on this. We are looking to identify one new priority in each of the following categories:

• Patient safety,
• clinical effectiveness and
• patient experience.

We have shortlisted three possible priorities under each category and would like you to choose which of the three suggestions you think would be the most important for us to work on. If you have another suggestion for a category, which is not on the list, you will have the opportunity to submit your suggestion on the survey.

When we have gathered all the results, we will use them to help us make a recommendation to the trust board to decide on to what the priorities should be for 2012/13.
