Service users and carers invited to be involved in a serious incident review panel

The Trust is seeking to recruit additional members to a serious incident review panel.

The panel has been identified following discussions with our commissioners, who are the people who buy mental health services for the local population from providers such as our Trust. The aim is to engage service users and carers in providing feedback on our reviews of serious incidents, which can support the improvement of our reviews and quality improvement for our services.

It is intended that the panel will meet approximately once every two months, it will review anonymised review reports and provide feedback to those who have undertaken the review and to the Trust investigations team in general. We are hoping that in future the role of the panel could develop further to provide more immediate feedback as review reports are completed.

Training will be provided to those involved in the panel.
