Community Mental Health Teams (CMHTs) are groups of mental health and social care staff that support people experiencing severe and enduring mental illness.
The teams are made up of a number of different staff. This can include Psychiatrists, Community Psychiatric Nurses (CPNs), Social Workers, Occupational Therapists, Support workers, Clinical Psychologists and Psychological Therapists.
A person referred to a Community Mental Health Team will often see more than one member of the team depending on the type of support he or she needs.
The team also offers carers assessments so that both carer and service users get the right help. People using the CMHT services and their carers are also closely involved with the team when planning the care that their patient receives.
