Community Rehabilitation Service

Our Community Rehabilitation Service provides intensive support to older people within the community.

The service provides home visits to people following their discharge from Assessment Units. We offer ongoing support that meets both the practical and emotional needs of the patient.

We also visit community patients in crises who may have refused help for their mental health problems in the past. Refusing help has often meant they have had to go into hospital for treatment.   For these patients, we spend time establishing a rapport and building a relationship, then we introduce further help on a gradual basis. We find this approach is nearly always successful.

 

Ongoing Support

We introduce people to resources in their local community. This could be leisure facilities, a ring and ride service or a day centre. We often accompany our service users to these places, to help them to get involved and to encourage people them to live a  more independent and active lifestyle.

We also make sure that people understand how to take their medication if they need to and offer support such as visiting on a daily basis to prompt them to take it if necessary.

Support for Carers

We provide home respite for people with dementia. This gives carers a break, and also helps patients to get used to accepting help from people other than their carers.

The service also runs two carer support groups for carers of people with dementia and three social support groups in the community for patients to attend on a weekly basis.
