Advocate An advocate will offer mutual support and help you to speak up, get your views heard and your needs properly met. Advocates are independent; they will help to represent your views but they won’t make decisions for you.

Care Coordinator This person will be your main contact. They will ensure you get the care you need

Care Plan This is where we record the care, support and treatment you need

Carers Assessment Process of assessing the needs of the carer and how we can support them

Crisis Plan A plan of what needs to happen in an emergency

GP General practitioner. You would see your doctor if you were physically unwell

Integrated Community Services This service is designed to give specialist care and support to those with complex mental health needs

Mental health professionals  These are the medical staff who make up the Integrated Community Service team

Recovery You will be able to manage the challenges of everyday life. Your recovery journey is very personal to you
