Support after discharge from our service

When our service users reach the end of their journey through the Integrated Community Care and Recovery Service and are discharged, they will still be able to access support.

At this point, they will be able to manage their own needs, and their care coordinator will write to their GP who will manage their care.

However, if needed service users can be referred back to mental health services or could self-refer if they have a rapid re access plan so that they get the support they need quickly.

There are many organisations that can help and support service users to continue their recovery, following discharge from the Integrated Community Care and Recovery Service.

Our team will access groups or activities that service users are interested in, and help them to overcome the challenges they may face.

Our Recovery College for All delivers sessions to help serice users learn about recovey and to ook after thier wellbeing.
