A health action plan identified the patient’s health needs, what will happen about them and what they need to do who will help and when this will be reviewed.
At BSMHFT, we are committed to ensure we are building the right support for people with a learning disability and autistic people. The plan will help to strengthen community support for people with a learning disability, and or autistic people and reduce reliance on mental health inpatient care.
