What is the dynamic support register?
The dynamic support register (DSR) is a proactive tool used by people working in health and social care services in England to identify and support people with autism and/or learning disabilities who are at risk of being admitted to hospital due to mental health challenges.
It is a live, regularly updated list that helps professionals coordinate care, plan early help and ensure that people receive the right support in the community. By tracking individuals’ needs and risks, the DSR aims to prevent unnecessary hospital admissions and promote person-centred, community-based care.
How will being on the dynamic support register help me?
We want to ensure that the dynamic support register helps you get the extra help you need, when you need it. It means that people will work together to find out what extra help and support you need to be safe at home.
How do I get onto the dynamic support register?
The people working with you will ensure a person-centred approach is adopted – that means you should be involved when competing the referral form to go on the register.
It’s good to complete the form with someone who knows you well. Some of the questions will be about:
- where you live and if you live with anyone,
- things that make you happy or upset,
- any other plans that may be in place, such as education, care, or health plans, and
- how best to keep you and family safe.
The form can be completed either with a healthcare professional or you can complete it with someone who knows you well.
